I had two interesting YASD's in Nethack:
First time I ever reached the castle, I stood under the drawbridge when I played the tune. Which always results in a nice instakill.
A few years later I reached the astral planes for the very first time, where I promptly met famine. Who hit me a few times, while I was trying to kill him. Starving me, steps away from victory.
