No overworld, immediately into the dungeon
Full diagonal movement(fuck brogue)
armor that doesnt work retardedly like in dnd(are you wearing a full plate harness? you are very hard to hit!)
high numbers of weapons and equipment (like the nethack's pole-weapons, similiar in practice but differentin name/description)
encumberance instead of inventory limit(gotta drop that axe to get that ring!)
weapon usage makes sense(spear makes you hit 2 tiles, axe hacks 3 tiles in front of you, blunt has a chance to stun etc..)
no sudden difficulty spikes(fuck you dcss)
lastly no how could i've known that?! deaths(fuck you nethack)
of course i dont expect all these expectations to be met but still..

It's violating some from your POV for sure.
Also turn off OOD monsters.

T.O.M.E. might fit the bill. There is an overworld though. But you start off right in the dungeon for a lot of the races you play.
